The short answer

Cold outreach gets ignored because it tries to buy attention with volume, and volume is the cheapest kind. The message that gets a reply leads with relevant proof, names a real problem, and asks for nothing on the first touch.

We ran the numbers on a campaign once and they were grim in the usual way. Thousands of emails, near silence. The few replies all came from a handful of messages that had one thing in common. They were not really cold.

Each of those had a line of proof in it. A result from a company the reader recognised. A specific problem we knew they had, named plainly. No template energy, no spray. The rest of the campaign had paid for attention with sheer volume, and volume is the cheapest currency there is, which is why it buys the least.

The lesson was not send more. It was earn the open. One message carrying real proof beats a thousand carrying a calendar link.

This is the same divide that runs under everything: paid attention versus earned attention. Volume is just a slower discount that buys nothing. The fix is to lead with what you have actually done, the way a good case study does.
